DESCRIPTION This study-unit is targeted for qualified, licensed and practising midwives to further their knowledge, skills, and experience concerning the physical and psychological implications of perineal trauma for women; means of preventing perineal trauma in the antenatal, intrapartum period and perineal care in the immediate postpartum period. The study-unit will revisit the anatomy and physiology of the perineum, pelvic floor and anal canal. It will also cover the indications for an episiotomy, when to suture and when not to suture and when to refer accordingly.

Study-Unit Aims:

The aim of the study-unit is to improve perineal assessment and suturing skills based on current evidence and to minimize postpartum morbidity.

Learning Outcomes:

1. Knowledge & Understanding:

By the end of the study-unit the student will be able to:
- Understand the anatomy and physiology of the pelvic floor structures and their significance to trauma sustained during childbirth;
- Discuss the role and responsibility of the midwife when completing perineal repair;
- Describe the choice of anesthetic, and the procedure for perineal infiltration;
- Identify current evidence for the choice of material and techniques when performing perineal repair;
- Recognise and treat postpartum complications and refer when necessary;
- Describe the documentation requirements of episiotomy and suturing; and
- Discuss perineal and pelvic floor care women require during pregnancy and following childbirth.

2. Skills:

By the end of the study-unit the student will be able to:
- Apply measures to prevent perineal trauma in the antenatal and intrapartum period;
- Provide perineal care in the immediate postpartum period;
- Recognise, assess, and classify perineal trauma;
- Recognise and refer 3rd and 4th degree perineal tears;
- Repair 1st and 2nd degree perineal tears, episiotomy and labia tears; and
- Competently perform episiotomy.
