DESCRIPTION This study-unit will address the concept of chronicity through an examination of its underlying features and other interrelated elements and the impact of these on the management of chronic illness. Chronic illness management will be presented through a framework that supports an interdisciplinary approach to holistic care and which places an emphasis on self-management. Furthermore, the role of the nurse in meeting the varied needs of the individual and his/her family throughout the stages of illness and in the diverse settings will be highlighted.

Study-Unit Aims:

The aim of this study-unit is to assist the student to acquire an understanding of the concept of chronicity and the experience of living with a chronic illness, including an identification of needs. This, together with a critical reflection on present day healthcare systems, should enable the student to develop an effective and holistic care plan that revolves around self-management and that is actuated through a delivery system that is based on interdisciplinary collaboration.

Learning Outcomes:

1. Knowledge & Understanding:

By the end of the study-unit the student will be able to:
- Demonstrate a knowledge and understanding of the concept of chronicity and interrelated components and the implications of these on the individual with a chronic illness and his/her family;
- Demonstrate knowledge of the common chronic health conditions and their medical management;
- Identify physical, functional, psychosocial and spiritual/existential needs of individuals living with chronic illness and their families, in consideration of any relevant ethical and legal implications; and
- Discuss whether current healthcare systems effectively satisfy the needs of the chronically ill individual and his/her family.

2. Skills:

By the end of the study-unit the student will be able to:
- Develop a self-management programme based on the characteristics and needs of the individual with chronic illness and the family;
- Utilise critical thinking skills to analyse the healthcare system and service provision, in relation to the specific needs of individuals with chronic illnesses and their families; and
- Address the contributory role of the nurse as a member of the interdisciplinary team in caring for the individual with chronic illness and the family.
