Abstract: Ehrat (1983) pointed out that politics is inherent in health care delivery because health care involves multiple special interest groups all competing for their piece of a limited pool of resources. At the Thirtieth World Health Assembly in May 1977, W.H.O. Member States decided that the main social target of governments and W.H.O. in the following decades should be for all citizens of the world to attain by the year 2000 "a level of health which will permit them to lead a socially and economically productive life". At the thirtieth session of the Regional Committee in Fez in September 1980, Member States of the W.H.O. European Region had also approved their first common health policy: the European strategy for attaining health for all. This called for a fundamental change in countries' health policies and urged that very high priority be given to health promotion and disease prevention; that all sectors with an impact on health take positive steps to maintain and improve health; that greater stress be placed on the role individuals, families and communities can play in health development; and that primary health care should be the major approach used to bring about these changes. It also called for the formulation of specific regional targets to support the implementation of the strategy. For Malta the main aim of the national health strategy, which formed an integral part of the socio-economic development plan for 1981-1985, was to ensure that all Maltese citizens received the best affordable health care; the immediate objective was to consolidate recent achievements and expand the primary health care, geriatric and hospital services. On the 5th December 1970, Malta decided to conclude an Agreement establishing an Association between the European Economic Community and Malta, in accordance with Article 238 of the Treaty establishing the European Economic Community. The aim of the Agreement was to progressively eliminate obstacles as regards the main body of trade between the European Economic Community and Malta. This was the first step towards the consolidation of political co-operation between Malta and the Common Market which would help to improve our country’s technical, financial and economic structures.

    <description>Title: Are we meeting mothers' expectations at time of birth?
Abstract: Mothers have certain expectations about their antenatal care and delivery. Their satisfaction depends whether these expectations are met or not. As would be expected, these expectations and needs differ from one mother to another and so it is always difficult to meet everyone's satisfaction. Childbirth is a life event and it should provide the mothers with a sense of personal fulfilment and satisfaction. Health care providers should be aware of the events which provide the mothers with full satisfaction of their childbearing experience. This study was envisaged so as to give mothers the opportunity to express their views regarding the service which should make us aware if we are meeting their expectations and if not what changes should be implemented. The study involved 200 mothers randomly selected during September 1994, who delivered normal or by emergency caesarean section so that they had the possibility to spend more time in labour ward and so could express their views better. The results show that the absolute majority (98.0 %) of the mothers were satisfied with care provided, but their views made us aware about areas on which we should improve.

    <description>Title: Management of lung cancer : an analysis of the quality of service provided to people suffering from lung cancer
Abstract: This study was devised to examine the quality of service that is being provided to those who are suffering from lung cancer. Repeated studies have shown that when treating such patients in acute medical wards too much emphasis is given to the patient's disorder rather than the person with the disease. Apart from being costly, this approach ignores the psychological trauma that both patients and their nearest ones suffer as a result of the condition. While there seems to be no shortage of diagnostic and therapeutic facilities, other, perhaps more. relevant needs are not being met. This project was carried out by studying the records of 5 deceased patients who had suffered from this condition between 1993 – 1994. Although by no means representative, the results of this study shows that many of the procedures that are currently done on an In-patient basis can be just as effectively done from an Out-patient clinic or as day cases. Moreover, many of the diagnostic tests that were done did neither changed diagnosis nor prognosis. Besides, management of lung cancer even at the advanced stage does not require acute medical care for it was shown that it can be done in other less acute units just as well. What is needed is more attention to the psychosocial aspects of care, better discharge planning and community support. Together, these may provide a more satisfying and less expensive service.

Abstract: Cost effectiveness of Laboratory Services at Zammit&#xD;
Clapp Hospital&#xD;
&#xD;
Zammit Clapp Hospital is a 60 bed geriatric hospital for&#xD;
the treatment of acute cases of elderly patients mainly&#xD;
candidates for rehabilitation. Essential Services for&#xD;
Rehabilitation, such as Physiotherapy and&#xD;
Occupational therapy are included in the hospital&#xD;
facilities.&#xD;
The decision whether or not to set up Laboratory&#xD;
Services at Zammit Clapp Hospital fall in a grey area. It&#xD;
was decided, after some considerations, that a small&#xD;
laboratory be established catering for a selected&#xD;
number of laboratory tests. The clinical professionals&#xD;
welcomed this service as being more accessible and&#xD;
providing a quicker benefit to the patients. The costs of&#xD;
running this laboratory need however to be carefully&#xD;
explained and its real benefits measured by accounting&#xD;
statistics and auditing techniques.&#xD;
The alternatives of using the services at St Luke's&#xD;
Hospital, or at private laboratories in terms of cost and&#xD;
availability were investigated in a scientific manner. A&#xD;
new proposal by a comprehensive private clinical&#xD;
laboratory service (Health Science and the&#xD;
Environment Laboratory Services) was compared with&#xD;
the present available Systems.&#xD;
The project investigates the following systems:&#xD;
1. The need of a Laboratory Service&#xD;
2. Achieving the ideal Laboratory Service&#xD;
3. Data Collection to establish the costs incurred&#xD;
over the year 1994 at Zammit Clapp Hospital for&#xD;
Laboratory Services&#xD;
4. The Acquisition of a complete proposal from a&#xD;
private Laboratory for a comprehensive&#xD;
Laboratory Service for Zammit Clapp Hospital&#xD;
5. Comparison of Cost Effectiveness&#xD;
6. Recommendations&#xD;
