dc.description.abstract | A poster presentation about Improving patient monitoring of methotrexate across transitional care settings. Introduction: Rheumatology patients are prescribed disease modifying antirheumatic drugs (DMARD) alone or in combination with biological DMARDs. An interprofessional collaborative team approach, based on an efficient monitoring process is essential, to maximize treatment effectiveness and ensure patient safety particularly as patients transition across different care settings. The pharmacist is a key member of the team positioned to ensure patients’ safety as they transition between primary and secondary care settings. Aim: • To assess compliance towards monitoring needs and laboratory blood tests of rheumatology patients who are treated with methotrexate. • To assess rheumatology patients compliance to prescribed treatment for methotrexate. | en_GB |
