DESCRIPTION This unit of study will address nursing actions that are required to meet the needs and individuals who have a chronic condition or a terminal illness. itogether with the needs of their carers. The emphasis will be mostly on self management using a holistic approach to care. an emphasis will also be made on the importance of continuity of care between the hospital and the community.

Topics that could be used as a point of reference for this unit include: osteoporosis, multiple Allergies, Lupus and other Chronic conditions

Study-unit Aims:

The aim of this unit is to assist student in acquiring an understanding of the nature of chronic illness and terminal care as well as be able to identify the needs of these individuals and their families . This will enable them to become aware of the demands these illnesses bring on the individual, their families and the community. This will enable students to help these individuals in self management of their chronic condition.

Learning Outcomes:

1. Knowledge & Understanding:
By the end of the study-unit the student will be able to:

Demonstrate a sound knowledge and understanding of the concept of chronic illness;
Develop knowledge and understanding of chronic health conditions and the needs of these individuals and their families;
Examine and analyse the underlying theory and principles of selected health conditions and interventions including physical, psychosocial , spiritual and ethical and legal considerations.

2. Skills:
By the end of the study-unit the student will be able to:

Assess plan implement and evaluate nursing care for these individuals and their families . formulate a self management programme for patients with chronic conditions;
Develope critical thinking skills in order to assess plan implement and evaluate care given to patients experiencing diverse health related conditions;
Work as a contributory member of the nursing and multidisciplinary team while caring for patients in order to deliver holistic care within a caring and therapeutic environment.
