| DESCRIPTION | The study-unit aims to help the student to develop concepts and improve practices in the organisation and planning of holistic care delivery. This involves: - Making use of the nursing process; - Integrated care by a multi-disciplinary team; - Focusing on patient-centered care; and - Active patient participation. Study-Unit Aims: The study-unit aims to help the student to develop concepts and improve practices in the organisation and planning of holistic care delivery. This involves: - Being able to deliver holistic care appropriate to the patients' needs; - Being capable of working effectively within a multidisciplinary healthcare team; and - Addressing care delivery using a systematic approach which enhances active patient participation. Learning Outcomes: 1. Knowledge & Understanding: By the end of the study-unit the student will be able to: - Discuss the principles underlying the organisation of care delivery; - Demonstrate an understanding about the role of the nurse and other professionals in the multi-disciplinary team; and - Illustrate the difference between actual and potential problems that may arise during care delivery through the use of case studies. 2. Skills: By the end of the study-unit the student will be able to: - Develop a care plan; - Identify actual and potential problems within care; - Address identified problems in care delivery; - Prioritise interventions to address patients' needs; - Refer and delegate aspects of patient care accordingly; and - Evaluate the effectiveness of care delivery through the development of an actual care plan.
