| DESCRIPTION | The study-unit is held over one semester with a series of didactic lectures and practical simulation sessions. The teaching programme offered as an optional study-unit parallel to SUR5504 is particularly directed at those students who wish to eventually follow a clinical career in gynaecological practice. Minimally-invasive surgery through laparoscopy is part of the basic skill-requirements in the training of the gynaecological surgeon. It requires a specific learning path from open surgery utilizing different skills. The study-unit aims to help the student appreciate the increasing role of gynaecological laparoscopy in modern day clinical practice and develop the necessary psychomotor skills to perform minimally-invasive surgery. Study-unit Aims: The aims of the study-unit are to: • build on the surgical principles taught in other study units of the course specifically applying the general principles to gynaecological surgical practice; • provide instruction and practical training to perform advanced laparoscopic suturing techniques for gynaecology trainees as recommended by the European Academy of Gynaecological Surgery (EAGS) in the Gynaecological Endoscopic Surgical Education and Assessment programme (GESEA), which is aimed to increase the practical skill level needed by endoscopic surgeons to correctly perform endoscopic procedures; • introduce the learner to laparoscopic suturing and knot tying techniques of GESEA level 2 – advanced skills, which includes intra-corporeal laparoscopic suturing with non-dominant hands; as well as continuous sequences (Greek suture); • introduce the learner to safe practice and principles of laparoscopic surgery for Tubo-ovarian pathology. Learning Outcomes: 1. Knowledge & Understanding By the end of the study-unit the student will be able to: • explain laparoscopic suturing and knot tying techniques of GESEA level 2; • explain the surgical anatomy of the pelvic organs, including detailed anatomy of pelvic vasculature, adnexa and supporting structures to the uterus (Round and uterosacral ligaments); • describe the peri-operative care; • describe the indications and contraindications of gynaecological surgeries; • identify the risks of surgeries; • identify the indications which require blood transfusion; • describe postsurgical complications and indications for admittance to the Intensive Care; • explain how gynaecological surgery can influence sexual function and psychosexual outlooks; • describe adnexal surgery for benign conditions (ectopic pregnancy, ovarian cysts, ovarian torsion, tubal disorders and tubo-ovarian abscess). 2. Skills Technical Skills will include: • Stitching Greek Suture - Continuous intracorporeal Suturing; • Stitching and knot tying with left hand (or non-dominant hand) - intracorporeal Suturing. Non-technical Skills will include: • Use of appropriate energy devices and cold instruments (scissors/graspers) to undertake tubal surgery (salpingostomy/salpingectomy), ovarian cystectomy with cyst wall stripping, ovarian stroma closure and/or suspension (suturing); • Maintain effective communication with the gynaecological patient and relatives according to the principles of shared decision making and informed consent; • Documents this communication accurately; • Perform teamwork with effective communication within the health care team; • Perform an effective pre-operative assessment in liaison with other healthcare specialists of the surgical team, e.g. anaesthetist; • Ensure a suitable reduction in the risks associated with gynaecological surgery; • Ensure appropriate patient positioning for dedicated gynaecological/pelvic including Lloyd-Davis & Trendelenburg positions. • Provide adequate therapeutic interventions to treat post-operative pain in the gynaecological patient, e.g. local anaesthetic infiltration, laparoscopic TAP blocks; • Draw up a regimen for urinary catheterisation after gynaecological and urological surgery; and • Anticipate surgical thromboembolism and institute timely prophylactic and therapeutic interventions.
