Examiner's confidentiality declaration

The Confidentiality Declaration is to be filled in when an external examiner or a supervisor not employed by the University is appointed to examine or supervise dissertations, projects, or theses because they may contain confidential or sensitive information.

In such cases, the examiner/supervisor must treat the information as confidential. There should only be one signatory to this declaration.

Take note

Apart from the required information requested in the document, all sections highlighted in yellow are to be filled in. Examiners and supervisors employed by the University need NOT sign the declaration since they are bound by confidentiality through their Collective Agreement or Employment Contract. Nonetheless they should be made aware about the confidential nature of the material. For information on procedures for processing the 'Examiners Confidentiality Declaration Form', one is to contact the Faculty Officer and the Departmental Secretary concerned.
