Abstract: The impact of medicine prices on accessibility significantly influences public health outcomes. High medication costs can restrict access, leading to delayed or inadequate treatment and compromising health outcomes. Understanding these dynamics is crucial for developing policies and strategies to ensure affordable access to essential medicines, thereby promoting equitable healthcare. Potential strategies include price regulations, subsidies, generic drug promotion, and international pricing agreements. Addressing medicine pricing issues can enhance healthcare accessibility and public health outcomes. This project aims to assess the influence of medication costs on accessibility, identify barriers posed by high prices, and propose solutions to improve affordability and equitable access to essential medicines. The methodology involved a systematic review of peer-reviewed journal articles published from January 2017 to July 2024, following PRISMA guidelines and utilizing thematic analysis. Searches were conducted in key databases such as PubMed/Medline, Google Scholar, and Hydi using keywords related to medicine availability and affordability. From an initial pool of 1,245 records, 22 studies met the inclusion criteria. Data were manually extracted using a standardized form, and a narrative synthesis approach was adopted due to the heterogeneity of the studies. Thematic analysis identified key themes including factors influencing medicine availability, economic barriers to affordability, policy impacts, and regional variations. Study quality was assessed based on design, sample size, and data collection methods to ensure reliability and validity. The studies followed methodologies established by the World Health Organization (WHO) and Health Action International (HAI) to assess the accessibility of both brand-name and generic drugs in public and private healthcare sectors. Results indicate significant variations in drug availability and affordability, with many regions (n=15) struggling to ensure consistent access to essential medications, particularly in public hospitals. The findings highlight the influence of local economic factors, healthcare policies, and pricing strategies on the accessibility of vital medicines. The study sheds light on factors affecting medicine availability and affordability, revealing complex interactions between supply chain challenges, economic obstacles, regulatory frameworks, and regional differences. Effective policy measures should be multifaceted and tailored to local conditions, including improved supply chain management, transparent pricing, broader insurance coverage, and increased competition in the pharmaceutical sector. Significant regional disparities, especially in low- and middle-income countries, necessitate targeted interventions to strengthen healthcare infrastructure and policies. Coordinated efforts from governments, international bodies, and industry stakeholders are essential to ensure fair access to essential medicines, underscoring the need for ongoing research and policy adjustments.
