| Abstract: | The CRISO project (“Coronary Risk Prevention in Siblings and Offspring of Patients with Premature Coronary Heart Disease”) aims to develop a preventive program for a potential high-risk group. The CRISO program facilitates opportunistic screening, enhances professional contact, and encourages first-degree relatives to seek cardiovascular risk assessment and support in risk factor modification. This case report presents a 62-year-old non-smoker Maltese man attending the CRISO program with no prior history of cardiovascular disease or type 2 diabetes. He had a sedentary lifestyle but maintained a good diet. During assessment, his office blood pressure was 137/83 mmHg, and his heart rate was 83 bpm. While on statin therapy, his lipid profile showed a total cholesterol of 4.9 mmol/L, LDL-c of 2.7 mmol/L, HDL-c of 1.4 mmol/L, and triglycerides of 1.9 mmol/L. His mean HbA1c was 5.6%, and serum uric acid measured 449 μmol/L. He had a BMI of 33.6 kg/m² and a waist circumference above ESC guideline recommendations (>94 cm for European men). His SCORE2 estimated a 10-year risk of myocardial infarction, stroke, or cardiovascular death at 7.3%, classifying him as high-risk (50–69 years; 5 to <10%). |
