Abstract: Early-stage cervical cancer poses unique challenges for women of reproductive age who wish to preserve their fertility. This article provides a comprehensive overview of fertility-preserving treatment strategies, focusing on surgical options which spare the uterus, such as conization and trachelectomy, as well as ovarian transposition. It examines the criteria for selecting appropriate surgical candidates, the role of accurate clinico-radiological staging, and the importance of thorough counselling by an experienced multidisciplinary team. Additionally, it explores assisted reproductive technologies that support fertility preservation including oocyte, embryo, and ovarian tissue cryopreservation. By emphasizing individualized treatment plans, we underscore the necessity of balancing long-term oncological outcomes with fertility preservation to enhance the quality of life for cervical cancer survivors.
