Abstract: A poster presentation regarding pharmacist intervention in patient monitoring in a psychiatric hospital. Introduction: Psychiatric patients are a high-risk patient population posing several challenges for safe and effective provision of pharmaceutical care.1Psychotropic medications often have a low therapeutic index and polypharmacy is common. Pharmacists have the adequate competence and ability to influence prescribers to alter prescribing practices thus bridging gaps between research and practice. Aims: To develop a ‘Patient Monitoring Tool’ to evaluate the quality of pharmaceutical care provided to patients in a psychiatric setting and to determine benefit from ward-based pharmacist services in this setting.
