Abstract: Background and Objective: Patients on warfarin are ideal candidates for medicine use review (MUR) due to treatment complexity. The objectives were to develop and implement a pharmacist-led MUR for patients on warfarin, assess patient knowledge, adherence and INR control, address identified risks of treatment and evaluate patient perception of the service.
