Abstract: Numerous predictive algorithms and nomograms have been pro- posed in attempts to individualize PHT dosing regimens. Software packages, based on Bayesian estimation, able to handle complex multidose regimens and fitting of clinical data, are now extremely common and permit rapid determination of appropriate individualized drug dosing even when data are sparse.
