All students are required to complete and submit the Occupational Health Protocol (OHP) via the online  Google Form.

Following submission, an appointment with the doctor must then be arranged through the Booking Page.

Students are advised to:

  • Read the Google Form carefully and follow all instructions provided.
  • Upload the required documents as specified.
  • Submit the Google Form as soon as possible.

Occupational Health Assessment screenings will take place at the Department of Public Health, Faculty of Medicine and Surgery. Upon confirmation, students are expected to strictly adhere to the assigned date and time.
